How this works

Score each measure below from 0 to 4. Each score is converted to points using score ÷ 4 × weight, and the weighted points add up to a total out of 100.

0consistently absent or harmful
1rare, mostly prompted, quickly reversed
2inconsistent, mixed evidence
3usually present, occasional failures
4consistently present, self-initiated, durable

Score only what you can actually observe — behavior, words, and patterns over time — not guesses about why the other person does what they do. Every measure here applies to both partners equally; a healthy relationship is a system, not one person grading another.

Score it twice — once for the last 12 months on average, and once for the best recent month. A big gap between the two suggests episodic stabilization, not real repair.

These are observable, not inferred — things either did or didn't happen, regardless of the stated reason behind them. If any are true, the result is go, or separate while evaluating — regardless of the total score. Check any that apply.
